The type of information we collect depends on the products and services you are interested in and whether you are the proposed insured. For example, we may collect personal identifying information such as your name, address, and telephone number, and we may collect other personal information such as your health, financial, and employment history. We may gather information from you in an application or claim form, in writing or by telephone required medical exams or test. We may also acquire information form other sources, such as medical professionals and consumer reporting agencies. While most information is collected at the time of application or claim, additional information may be required of you request changes to your contract or other service. We may also have personal information from and past transactions with us, such as information about you insurance policies, premiums, and claims.             Information we may disclose to third parties Nissly and Nissly regards all of your personal information as confidential. However, in the course of conducting our business there are circumstances in which we may disclose information we have about you to other parties. These disclosures are only made as permitted by law, e.g.: re-insured, administrators, consults, and regulatory or governmental authorities.             Non-affiliated parties to whom disclosures may be made We may disclose nonpublic personal information about you to the following types of third parties, unless you tell us not to: Financial service providers such as life insurers, automobile insurers, insurance agents or providers. If you prefer that we not disclose nonpublic personal information about you to non-affiliated third parties, you may opt out of those disclosures. That is, you may direct us not to make those disclosers other than those that are expressly permitted by GLBA, HIPPA and their regulatory guidelines.             Information confidentiality and security We have and maintain policies and procedures to help us protect the confidentiality of your personal information during all stages of you relationship with us. We maintain physical, electronic and procedural safeguards to help us protect you personal information from unauthorized access and anticipated threats or hazards. Access to your information by employees and other representatives is restricted to those having a business need for such information. We maintain physical, electronic and procedural safeguards that comply with federal regulations to guard you nonpublic personal information.             Dispute Resolution Any controversy or claim arising out of or relating to our privacy policy, or the breach thereof, shall be settled by arbitration in accordance with the rules of a the American Arbitrators(s) may be entered in any court having jurisdiction thereof.             Reservation of the right to disclose information In connection with the potential sale or transfer of its interests, Nissly and Nissly reserves the right to sell or transfer your information including but not limited to your name, address, sex, zip code, state and country of residency and other information that you provide through communications to a third party entity that 1) concentrates its business in a similar practice or service; 2) agrees to be Nissly and Nissly successor on interest with regard to the maintenance and protection of the information collected; and 3) agrees to the obligations of the privacy act.         If you wish to opt out of such discloser to  non-affiliated third parties, please notify us in writing regarding your specific wishes.
